Dependent Head of Household Worksheet - 2025–2026 - (Dynamic Form)

The student will start this form and complete just information pertaining to them. It is then routed to the parent to complete their portion.

This form is for the collection of Department of Homeland Security or other U.S. citizenship/nationality documents from students who are unable to present their documents in person at the Financial, Billing and Enrollment Center or a Kent State Regional Campus student services area. The student must complete this form in the presence of a Notary, who must notarize it on the second page of the form.

The chart below shows the number of credits within each term during the summer semester that are needed for full-time status in order to obtain the maximum monthly housing allowance/stipend.

If you are looking to get other tuition funding, you may need to be full-time (ex. Ohio War Orphans Scholarship or Maximum Pell Grant). 

If you have questions about a specific funding source and the requirements for summer, please reach out to the designated department that administers those funds.
